McAfee's Host Based Security Software's Host-based Intrusion Protection System (HIPS) is the cause of more problems than I care to think.
When troubleshooting something strange, always check you HIPS (or equivalent virus scanning) logs.
Especially if that something weird throws permissions errors or no errors at all.
